A Special Needs Plan Is a Medicare Advantage Plan With an Eligibility Requirement
Worth being clear about this first, because it is easy to miss. An SNP is a type of Medicare Advantage plan. Enrolling in one means your coverage runs through a private plan rather than Original Medicare, with a network and the rules that come with it.

What the eligibility requirement buys you is a plan built around a specific group of people:
Care coordination
Every SNP includes it, which matters when several doctors are involved in your treatment.
A tailored formulary and network
The drug list and provider network are built around the condition the plan serves.
Sometimes extra services
Some plans cover more than a standard Advantage plan would for the condition they are designed around.

The Two Types of Special Needs Plans We Write
There are three kinds of Special Needs Plan. These are the two that apply to most people.
Find Out If You Qualify
A short conversation answers it. We will tell you:
Whether you qualify, and which of the two types applies to your situation
What verification would involve, and who needs to provide it
Whether a Special Needs Plan is actually your best option, or whether something else fits better
Or call (561) 446-2821 and we will work through it on the phone.
Qualifying for One Does Not Make It Your Best Option
This is the part that usually goes unsaid, and it is the reason to talk to someone who can see the whole market.

66 Special Needs Plans in Palm Beach County
55 regular Medicare Advantage plans in the same county
If you have diabetes, you are not choosing among 21 chronic condition plans. You are choosing among roughly 120 plans, and a condition-specific plan is not automatically the better one.
Worth knowing before you decide: chronic condition plans in this county carry a higher average maximum out of pocket than Special Needs Plans generally, around $7,529 against an average closer to $4,603, and 18 of the 21 carry no monthly premium at all.

Sometimes the condition-specific plan is clearly the right answer, particularly when the care coordination and tailored network genuinely fit how you are being treated. Sometimes a regular Advantage plan does better on cost and network. The only way to know is to compare both sets, which is what we do.
You Stay Enrolled Only While You Still Qualify
This is the most important thing to understand about these plans and the thing we see covered least often.

Your enrollment depends on continuing to meet the plan's conditions. If your Medicaid eligibility changes, or a chronic condition can no longer be verified, you do not simply stay on the plan. Coverage ends.

The good news is that losing eligibility generally opens a Special Enrollment Period, so you get a window to move to another plan rather than being left without coverage. The window has a deadline, though, and it is not a long one.

If you get a letter about your eligibility status, call us before you do anything else. This is a situation where acting quickly matters more than getting it perfect.

Four Protections Most People Never Hear About
These apply to Special Needs Plans and they are worth knowing, particularly if you are already in treatment.
Your costs are capped for chemotherapy, dialysis and skilled nursing care
A Special Needs Plan cannot charge you more than Original Medicare would for these services. For anyone managing cancer or kidney disease, this removes one of the bigger worries about moving to a private plan.

An approved treatment stays approved

If your plan authorizes a treatment, that approval has to remain valid for as long as the treatment is medically necessary. The plan cannot make you go back and get it authorized again.

Switching plans gives you 90 days of continuity
If you change plans while you are in treatment, the new plan has to give you at least 90 days before it can require a fresh authorization. This exists so that changing coverage does not interrupt care that is already underway.
Every SNP includes prescription drug coverage
All Special Needs Plans are required to include Part D. You will not need to add a separate drug plan alongside one.
